    • Company Name
    • Contact name
    • Phone Number
    • Serial Number
    • MAC Address
    • App Release Version
    • WearMF Version
    • Customer Configuration – if any
    • Type of Connection (WiFi, 2G/3G/4G Hotspot, etc.)
    • Problem description troubleshooting information / Steps taken
